Qualifications

Bachelor's or PharmD from an accredited school of pharmacy.
Postgraduate Year One (PGY-1) Pharmacy Residency or equivalent.
Washington Pharmacist License required at hire.
Within 2 years of hire: BPS certification.
Preceptor license upon hire where applicable.
3 years of pharmacy practice with at least 2 years of related experience in lieu of PGY-1.

Responsibilities

Provide collaborative practice services with the healthcare team.
Collaborate with physicians and staff to deliver high quality patient care.
Utilize registries and reports/tools for population management and education.
