Job summary

gobravvo is seeking a dedicated Primary Care Physician (PCP) in Orlando, FL to deliver preventive primary care for adults aged 65 and older. You will join a value-based care model with a small patient panel, supported by a collaborative care team and strong administrative support.

The role emphasizes high-quality, personalized care with no night or weekend shifts, offering partnership pathways and opportunities for professional growth in a patient-focused environment.

Qualifications

• Residency training in Internal Medicine, Family Medicine, or Geriatric Medicine.
• Board certification or eligibility in IM, FM, or Geriatric Medicine.
• Experience with value-based care (VBC) is preferred but training is provided.

Responsibilities

• Provide comprehensive primary care to a panel of adult patients, focusing on preventive medicine and chronic disease management.
• Manage patient panel with emphasis on seniors, coordinating care with a dedicated team.
• Develop individualized care plans for patients with multiple chronic conditions.
• Collaborate with in-house specialists (cardiology, podiatry) and holistic services as appropriate.
• Work with a care team including medical assistants, schedulers, and referral specialists.
• Incorporate value-based care principles to improve patient outcomes.
• Maintain precise and up-to-date medical records and documentation.
• Participate in regular team meetings to discuss care strategies and improvements.
• Educate patients and families on health maintenance and preventive strategies.
