UnitedHealth Group is seeking a Palliative Care Nurse to provide direct patient and family care through palliative care consultations. The role focuses on delivering comprehensive, ethical care for patients with serious illness and limited prognosis, guiding clinical decisions and care plans.

Responsibilities include thorough assessments, plan development, and coordination of multidisciplinary care. ACHPN certification is encouraged after two years of service; strong RN/APRN credentials and BCLS

Qualificações

• RN/APRN licensure in state of practice.
• Nurse Practitioner/CNS with prescriptive authority.
• BCLS licensure.
• 5+ years of clinical nursing experience.
• ACHPN certification encouraged after 2 years of employment.
• 1+ years as APRN in Hospice, Palliative Care, or Oncology (preferred).

Responsabilidades

• Provide direct patient and family care through palliative care consultations.
• Deliver care based on palliative medicine practices across physical, emotional, spiritual and psychosocial domains.
• Gather information from medical records, family, and caregivers and perform comprehensive assessments.
• Develop and implement a care plan, including medication reconciliation and evaluation of outcomes.
• Support ACHPN certification after relevant experience.
