Qualifications

• Master’s degree in Nursing and NP licensure in the state where services are provided.
• Certified as a Nurse Practitioner by AANP or ANCC.
• Authorized to practice and prescribe as an NP, with DEA where required.
• Medicare billing privileges for Nurse Practitioners.
• 1+ years NP or 3+ years RN experience in geriatric, hospice or palliative care.

Responsibilities

• Provide palliative care in home and clinical settings in collaboration with physicians and medical directors.
• Perform Medicare-compliant assessments, schedule visits, and develop individualized care plans.
• Order and review treatments and tests, update plans with physician input.
• Complete documentation and chart reviews promptly and accurately.
• Coordinate care across providers and facilities; educate patients and families.
