Job summary

Aultman Health Foundation, Canton, OH is seeking an Advanced Practice Provider (CNP/NP or PA) for Home Palliative Care. The role involves conducting comprehensive home visits, coordinating care with physicians, and participating in quality improvement activities.

The ideal candidate has NP/PA certification, an ARNP license with prescriptive authority or PA license with DEA, and 3 years of nursing experience (hospice preferred). Travel within Eastern Stark County is required.

Qualifications

• Advanced practitioner training and certification from an approved NP or PA program.
• Current state license as ARNP with prescriptive authority or PA license with DEA certification.
• Three years of full-time nursing experience; hospice nursing preferred.
• Excellent interpersonal and oral/written communication; strong organizational and problem-solving.
• Current BLS certification.

Responsibilities

• Use advanced comprehensive assessment, diagnostic, treatment planning, implementation, and evaluation skills to provide nursing care.
• Document assessment, diagnosis, and management of treatment and care provided.
• Prescribe medications and treatments according to Ohio APN scope of practice and policies.
• Collaborate with Hospice and Palliative Care physicians on consultations, rounds and visits.
• Coordinate with other health care professionals to maintain continuity of care.
• Participate in education plans and QAPI improvement activities.
