Job summary

Optum, through Elite Palliative Care, seeks a Palliative Care Nurse to provide direct patient and family care via palliative consultations for patients with serious illness. The role supports patients who may not be hospice eligible while ensuring ethical and compassionate care.

Required qualifications include RN or APRN licensure in the state, prescriptive authority for APRN/CNS, BCLS, and 5+ years of clinical nursing. ACHPN is encouraged after 2 years.

Qualifications

• RN/APRN licensure in state of practice.
• Nurse Practitioner/CNS with prescriptive authority.
• BCLS licensure.
• 5+ years of clinical nursing experience.

Responsibilities

• Demonstrates effective and ethical use of palliative medicine practices in patient care based on multiple domains.
• Formulates an effective plan of care using palliative care knowledge and evidence-based interventions.
• Gathers information from medical records, family history, and caregivers for comprehensive assessment.
• Performs comprehensive assessments, interprets findings, reconciles medications, and evaluates outcomes.
