Job summary

Qualifications

• RN/APRN licensure in state of practice.
• Nurse Practitioner/CNS with prescriptive authority.
• BCLS licensure.
• Current CPR certification.
• Minimum 5 years of clinical nursing experience.
• Minimum 1 year as APRN in Hospice, Palliative Care, or Oncology.
• Advanced practice palliative care certification (ACHPN) preferred.

Responsibilities

• Provides direct patient and family care through palliative care consultations.
• Ensures specialty services for patients with serious illness with a prognosis of one year or less.
• Demonstrates ethical, evidence-based palliative medicine practices in patient care.
• Gathers information from medical records, family, and caregivers to assess patient needs.
• Performs comprehensive assessments and develops plans of care with therapeutic interventions.
