Job summary

UnitedHealth Group’s Casa de la Luz (part of LHC Group) seeks a Hospice Nurse Practitioner to provide consultation in palliative care, symptom management and supportive care for Hospice patients.

The role requires collaborating with Hospice Medical Director and interdisciplinary teams, conducting face-to-face visits, coordinating plans of care, documenting patient progress, and guiding families in self-care. On-site position in Mesa, AZ with competitive pay and benefits.

Qualifications

• Active unrestricted license in the state of practice (AZ) is required.
• Current certification as a clinical Nurse Specialist or Nurse Practitioner in the state of practice.
• Current certification by a national body such as ANCC, NBCHPN, or AANP.
• 5+ years nursing experience.
• Current driver's license and vehicle insurance, access to dependable transportation.

Responsibilities

• Makes face-to-face encounter visits within prescribed time limits to qualifying Hospice patients and reports clinical findings to the Hospice Medical Director.
• Consults with the Hospice Medical Director as needed and informs primary physician of recommended services.
• Initiates and coordinates the plan of care, documents problems, goals, interventions, and patient/family response to hospice care.
• Instructs and supervises the patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes and reports patient condition and needs to Administrator and Hospice Medical Director.
