Job summary

UnitedHealth Group invites you to join our Palliative Care Nursing team. As a Palliative Care Nurse, you will provide direct patient and family care through palliative consultations, focusing on patients with serious illness and a prognosis of one year or less who are not ready for hospice.

You will perform comprehensive assessments, develop care plans with an ethical approach, and collaborate with multidisciplinary teams to improve comfort and quality of life for patients and families we serve.

Qualifications

  • RN/APRN licensure in state of practice.
  • Nurse Practitioner/CNS with prescriptive authority.
  • BCLS licensure.
  • Minimum 5 years of clinical nursing experience.
  • Current CPR certification.

Responsibilities

  • Applies evidence-based palliative medicine practices in patient care across physical, emotional, spiritual, psycho/social, ethical, educational, safety and related criteria, appropriate to patient ages.
  • Incorporates palliative care knowledge to formulate an effective plan of care.
  • Gather information from medical records, family history, caregivers, and perform thorough specialty assessments.
  • Completes comprehensive assessment, interprets findings, initiates plan of care with therapeutic interventions, and evaluates outcomes.
  • ACHPN certification encouraged after 2 years of employment or with sufficient experience.
