Job summary

Heartlite Hospice, part of the LHC Group and a member of the Optum family, seeks a Hospice Nurse Practitioner to provide consultation in palliative and supportive care for hospice patients.

You will perform face-to-face visits, coordinate with the Hospice Medical Director, and collaborate with physicians to implement care plans. A valid state license, NP/CNS certification, and 5+ years of nursing experience are required.

Qualifications

• Active unrestricted license in the state of residence.
• Current certification as a clinical Nurse Specialist or Nurse Practitioner in the state of practice.
• Current certification by a national body such as ANCC, NBCHPN, or AANP.
• 5+ years nursing experience.
• Current driver's license and vehicle insurance or access to transportation.

Responsibilities

• Makes face-to-face encounter visits within prescribed time limits to qualifying Hospice patients and reports clinical findings to the Hospice Medical Director.
• Consults with the Hospice Medical Director as needed, informs primary physician of recommended services, and collaborates with other physicians as needed.
• Initiates and coordinates the plan of care. Documents problems, goals, interventions, and patient/family response to hospice care. Collaborates with patient/family, attending physician and other IDG members.
• Instructs and supervises the patient/family in self-care techniques when appropriate.
• Maintains accurate and relevant clinical notes and reports patient condition and needs to Administrator and Hospice Medical Director.
