Included Health is seeking a Population Health Strategy & Operations Senior Manager to optimize program operations and drive strategic execution across Care & Case Management and navigation teams. The role focuses on turning messy cross-functional work into actionable plans, using AI to boost team efficiency, and delivering data-driven performance insights.

The position reports to the Senior Director and requires senior-level collaboration, strong Excel modeling, and Looker-informed analytics.

Qualifications

• Seven+ years managing cross-functional projects in a fast-growing business.
• Operationally minded with ability to analyze a process and test improvements.
• Strong modeling skills: proficiency in Excel to build robust models.
• Proficiency with AI productivity tools to enhance efficiency.
• Ability to organize, delegate, coordinate and prioritize multiple tasks in a team.
• Willingness to mentor and coach employees.
• Excellent oral and written communications skills.
• Experience with data analysis (Looker preferred).
• Healthcare experience preferred.
• Bachelor’s degree required.

Responsibilities

• Lead operational testing and experiments for CCM and navigation teams, including AI pilots.
• Manage cross-functional projects with timelines, milestones and KPIs.
• Own daily and weekly business readouts with the data team for visibility into metrics.
• Lead daily huddles for the CCM leadership team and act as a liaison with clinical services.
• Conduct deep dives into operations to identify bottlenecks and test solutions.
• Own executions of Population Health margin projects and improvement initiatives.
• Identify opportunities to use AI productivity tools and automation.
• Collaborate with EPDD product partners to shape features that fit team needs.
• Anticipate risks and develop mitigation strategies.
