Job summary

Covera Health is seeking a Vice President of Population Health to lead the Protect Seniors program, spanning Customer Success and Operations. You will own the performance of a proven population health model and serve as the primary relationship owner for health plan and PCP partners, influencing strategy with the Executive Leadership Team.

Role requires deep Medicare Advantage fluency, ~50% travel, and the ability to drive growth, operational excellence, and long-term customer success across

Qualifications

• 10+ years healthcare experience with healthtech start-up, consulting, or enterprise healthcare background
• Deep Medicare Advantage experience and understanding of value-based care/risk sharing
• Proven track record collaborating with payors, providers, or both on high-visibility initiatives
• Background in customer success, account management, or population health operations with multi-stakeholder accounts
• Demonstrated ability to influence and drive change across teams and levels
• Strong written and oral communication skills; ability to convey complex topics clearly
• Hybrid combination of strategic thinking and execution orientation to drive impact
• Humility, high EQ, and ability to build trust with diverse partners
• Comfort working in a startup environment with ownership and problem-solving mindset
• Experience engaging with primary care physicians and PCP leadership for provider engagement

Responsibilities

• Own end-to-end performance of the Population Health value chain from data intake to outcome validation
• Build and manage relationships with Customer executive sponsors and align on strategic objectives
• Partner with Growth/Commercial to align go-to-market strategy with delivery capabilities
• Drive population health adoption across markets engaging ACOs PCP groups and radiology facilities
• Define and execute Population Health strategy with cross-functional teams across product, tech, and analytics
• Serve as primary contact for PCP partners and ensure providers are active participants
• Oversee deployment of Population Health programs from pilots to scalable partnerships
• Collaborate with internal teams to plan Population Health priorities across functions
• Establish KPIs for program success and monitor performance with analytics
• Maintain high-trust relationships with key accounts through in-person engagement
• Participate in interviews and cross-company projects after onboarding
