Thyme Care Inc. is seeking a Complex Oncology Nurse Navigator to support members diagnosed with cancer. You will conduct clinical assessments, coordinate care, and educate members and caregivers by phone to ensure seamless oncologic care.

This role can be remote or hybrid-based in our Nashville office, requiring strong clinical focus, cultural competence, and a member-first approach. You will collaborate with care teams and payers to optimize outcomes.

Qualifikationen

• Active RN license required (compact) with ability to obtain state licenses as needed.
• BSN preferred; ADN or higher acceptable with experience.

• Conduct clinical assessments and monitor health changes for oncology patients.
• Coordinate care plans and transitions across care teams.
• Educate members and caregivers about diagnoses and treatments by phone.
• Handle clinical escalations and support care-team decisions.
