Job summary

Thyme Care is seeking an Oncology Nurse Navigator to provide triage, support, and education to members and their care networks via phone and email. You will perform clinical assessments under the Nurse Team Lead, oversee member health, and facilitate end-of-life discussions within a compassionate, member-centered framework.

Role emphasizes symptom management and longitudinal care coordination, with shifts 8:30 AM–5 PM EST or 11:30 AM–8 PM EST, and a focus on reducing readmissions through

Qualifications

• Active, unrestricted RN license is required.
• Degree in Nursing (ADN or BSN) required; BSN preferred.
• Compact and single-state licensure in NY and/or IL is preferred.
• At least 5 years of nursing experience, including 3 years in solid tumor oncology.
• Oncology-related certifications such as OCN, AOCN, AOCNS, or CCM are required or obtained within 2 years of hire.

Responsibilities

• Provide triage, support, and education to members and their care network via phone and email.
• Conduct comprehensive clinical assessments under guidance of Nurse Team Lead.
• Oversee member health and facilitate end-of-life care discussions.
• Engage with members to address clinical issues and manage escalations.
• Coordinate care and provide clinical support across the oncology care continuum.
• Collaborate with payers and primary/oncology providers to optimize care and reduce readmissions.
• Address social determinants of health with care team partners.
