Thyme Care is seeking an Oncology Nurse - Transitions of Care to join our frontlines serving members diagnosed with cancer. You will conduct clinical assessments, coordinate care, and educate members and caregivers by phone to support their oncology journey.

You must hold an active RN license with oncology or CCM experience. This role supports a hybrid/remote schedule from Nashville, with a pay rate of $46.35/hour and options to collaborate with payers and providers.

Qualifications

• Active, unrestricted RN license required.
• 5+ years nursing experience with 3 years in oncology or case management.
• Certified Case Manager (CCM) designation preferred.
• Able to manage multiple tasks and documentation under pressure.

Responsibilities

• Conduct clinical assessments and monitor changes in health.
• Coordinate care and transitions for oncology patients.
• Educate members and caregivers over the phone.
• Handle clinical escalations and referrals.
• Support discharge planning and follow-up care.
• Document consistently and follow policies.
