Job summary

Thyme Care is seeking a Complex Oncology Nurse Navigator to support members diagnosed with cancer. You will conduct clinical assessments, coordinate care, and educate patients and caregivers over the phone, reporting to the RN Care Team Lead. Role permits remote or hybrid work from our Nashville office.

You will manage high-acuity oncology patients, follow care pathways, and collaborate with payers and providers to optimize outcomes. Strong clinical focus and cultural competence are essential.

Qualifications

• Active RN license in good standing with oncology experience.
• Minimum 3–5 years of oncology nursing experience preferred.
• Oncology certifications (OCN/AOCN/AOCNS/CCM) preferred.

Responsibilities

• Conduct telephonic assessments and care coordination with members.
• Escalate clinical issues and support the care team.
• Assist with discharge planning and follow-up care.
• Educate members and caregivers about diagnosis and treatment.
