Job summary

Qualifications
- Must have 5+ years of nursing experience with minimum 3 years in adult oncology.
- OCN, AOCN, AOCNS, or CCM certification is required.
- Fluency in Spanish preferred.
Responsibilities
- Conduct clinical assessments and monitor health changes.
- Coordinate care and educate members and caregivers over the phone.
- Assist with care coordination after admissions.
