Job summary

PACS is seeking a Staff Occupational Therapist at Castle Hills Rehabilitation and Care Center to evaluate and treat residents, coordinate with physicians, and maintain documentation. You will develop individualized plans to improve mobility, strength, balance, and independence, while educating residents and families on home programs and safety.

You will supervise Certified Occupational Therapy Assistants and support staff, participate in care conferences, and ensure compliance with facility

Qualifications

• Bachelor’s degree in Occupational Therapy required; Masters/Doctorate preferred.
• Licensed as an Occupational Therapist in the state.
• Proficient with health record systems such as Net Health and PointClickCare.
• Strong communication with residents, families, and healthcare teams.

Responsibilities

• Evaluate residents to determine OT needs and rehab potential.
• Develop and update individualized treatment plans to improve mobility, strength, balance, and independence.
• Supervise COTA and trainees; provide training to facility staff.
• Document services timely and accurately in medical records.
• Educate residents and families on home programs, safety, and fall prevention.
