Job summary

Senior Friendship Centers Inc. is seeking a dynamic Nutrition Case Manager in Sarasota, FL. The role involves screening and assessment for home-delivered meals, supporting congregate dining sites, and coordinating nutrition programs across Sarasota County.

You will meet with seniors in homes to determine program eligibility, drive to multiple locations, and participate as on-call staff at meal sites. The role offers full-time hours and comprehensive benefits.

Qualifications

• Requires Case Management Experience.
• Management experience is strongly preferred.
• Experience presenting social, recreational, and educational programs to adults is desirable.
• Intermediate computer skills; proficient with Microsoft Office Suite.

Responsibilities

• Administer the DOEA 701A/701C assessment form electronically and complete all case file documentation for HDM and congregate meals.
• Initiate, coordinate, and monitor meal service plans with clients, acting as liaison with agencies.
• Contact home-delivered meals waitlisted clients to determine continued interest.
• Data entry, client reports, and verification of information; assist with meal orders and billing functions.
• Develop and promote relationships with service providers to support the organization's mission and nutrition outreach.
• Oversee daily operations of congregate meal sites, including setup, sanitation, and vendor coordination.
• Assist with Area Agency on Aging and Department of Elder Affairs requests and other administrative records.
