You Thrive Florida seeks a dedicated Case Manager to assess functional needs and coordinate community-based services for older adults in Brooksville, FL. Hybrid of home visits and office work, focusing on client self-sufficiency and coordinated care.

Responsibilities include reviewing care plans, conducting routine visits, and maintaining accurate records while collaborating with staff and community partners. Requires a Bachelor's in Human Services and valid Florida driver’s license.

Qualifications

• Bachelor’s degree in Human Services or related field.
• At least one year of Case Management experience.
• Valid Florida driver’s license.
• Background checks/clearance required.

Responsibilities

• Coordinate services to support client self-sufficiency and continuum of care.
• Conduct client visits and assessments to ensure service adequacy.
• Review care plans and perform semi-annual/annual reviews.
• Maintain updated client records and timely narratives/referrals.
• Engage in staff meetings and community outreach as directed.
• Ensure APS referrals are initiated and followed up as required.
• Communicate effectively with clients, families, and providers.
