Job summary

You Thrive Florida is seeking a dedicated Case Manager to coordinate community-based services for older adults. You will apply client-centered approaches to promote self-sufficiency and continuity of care, conducting home visits and updating care plans as needed.

Responsibilities include reviewing plans, collaborating with caregivers, and participating in outreach to expand Senior Services access in Florida communities.

Qualifications

• Bachelor’s degree in Human Services or related field.
• One year of case management experience.
• Valid Florida driver’s license and insurability.

Responsibilities

• Coordinate community resources to support client self-sufficiency.
• Conduct telephone calls and client home visits to assess safety and needs.
• Review care plans and conduct semi-annual/annual reviews.
• Maintain confidential client records and ensure data quality.
• Engage in community outreach to promote Senior Services.
• Complete APS referrals timely.
• Maintain professionalism and effective relationships with contacts.
