Optum Home & Community Care seeks an Advanced Practice Clinician (NP/PA) to provide Senior Community Care in field-based settings across long-term care communities and private homes. You will practice at the top of your license, perform comprehensive assessments, and drive patient outcomes with an integrated interdisciplinary team.

This role offers clinical autonomy, a flexible schedule, no call, and relocation or sign-on incentives.

Qualifikationen

• MSN or DNP from an accredited program.
• Active Nurse Practitioner license.
• AANP or ANCC certification in an NP specialty.
• Graduate of an accredited Physician Assistant program.
• Active Physician Assistant license.
• NCCPA certification.
• DEA licensure/prescriptive authority or ability to obtain.
• Collaborative practice agreement, if applicable.
• Valid driver’s license and reliable transportation.

Aufgaben

• Conduct comprehensive patient assessments.
• Manage acute and chronic conditions.
• Develop and oversee complex treatment plans.
• Lead advanced illness, goals-of-care, and advance care planning discussions.
• Collaborate with caregivers, facility teams, PCPs, and interdisciplinary partners.
• Educate patients and caregivers.
• Maintain accurate clinical documentation.
