Zusammenfassung

Optum Home & Community Care, part of the Optum family, is expanding the Senior Community Care program to provide integrated care in nursing homes and senior living settings. The Advanced Practice Clinician will collaborate with an interdisciplinary team to coordinate care for high-risk members and support their physical, mental, and social needs.

We offer a balanced schedule with no on-call, weekends, or holidays required, plus a $40,000 student loan repayment or a $30,000 sign‑on bonus.

Qualifikationen

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Current NCCPA certification and state licensure
• Active, unrestricted state licensure in the practicing state
• Active and unrestricted driver's license with reliable transportation
• Current DEA licensure/prescriptive authority or ability to obtain post-hire (not required for Patient Connect but needed for ISNP/IESNP)

Aufgaben

• Conduct comprehensive assessments including medical history review, vitals and physical exam
• Support members during a short term sub-acute rehabilitation episode
• Ensure accurate and complete documentation including ICD-10
• Communicate and collaborate with interdisciplinary care team
• Conduct advanced illness and advanced care planning conversations
• Provide counseling and education to patients and caregivers
