Job summary

Optum Home & Community Care is seeking an Advanced Practice Clinician to join the Senior Community Care (SCC) team. You will coordinate clinical care for high-risk members in nursing homes, senior housing and assisted living settings.

This role emphasizes a no on-call, no weekends, and no holidays schedule with a strong emphasis on teamwork and patient outcomes. Compensation ranges from $125,000 to $175,000 with a comprehensive benefits package, incentive programs, equity stock purchase options,

Qualifications

• Active and unrestricted licensure in the state you will be practicing in
• Current, unrestricted DEA licensure/ prescriptive authority or ability to obtain
• Nurse Practitioner or Physician Assistant degree or equivalent
• Access to reliable transportation beyond a driver’s license

Responsibilities

• Conduct comprehensive assessments including history, meds, vital signs, and physical exam
• Support members during sub-acute rehabilitation episodes
• Ensure accurate documentation including ICD-10
• Collaborate with interdisciplinary care team
• Conduct advanced illness and end-of-life conversations
• Provide counseling and education to patients and caregivers
