Job summary

Qualifications

• Master's Degree in Social Work from accredited institution.

• Ability to analyze, develop and manage change.
• Demonstrates ability to connect patients and families with necessary services.

Responsibilities

• Psychosocial Assessment and Interventions: assess risk factors and mobilize resources.
• Interventions for emotional, social, financial consequences of illness; connect to community resources.
• Discharge planning for complex patients; coordinate with care team.
• Serve as preceptor, mentor, trainer to staff.
• Promote safe care within budget and productivity constraints.
• Lead change management and organizational process improvements.
• Collaborate to formulate discharge plans and care needs.
