Job summary

CHRISTUS Health seeks a Hospital Social Worker to support patients and families across the continuum of care. You will conduct age-appropriate psychosocial assessments, guide care plans with the care team, and assist with discharge planning to ensure seamless transitions.

This role requires a Master's in Social Work, strong communication, and the ability to collaborate with physicians, families, and community services. Hospital setting with day shift is typical.

Qualifications

• Master's Degree in Social Work from accredited institution.
• Strong interview, assessment, organizational and problem-solving skills.
• Excellent interpersonal communication and negotiation skills.
• Ability to work with people of diverse backgrounds
• Ability to analyze, develop and manage change
• Ability to work independently and develop relationships with physicians, families, patients, interdisciplinary team and community agencies
• Demonstrates ability to connect patients and families with necessary services
• Maintain working knowledge of community resources and funding options

Responsibilities

• Perform psychosocial assessments and develop interdisciplinary care plans with patients and stakeholders to meet needs.
• Assist with discharge planning and link to post-acute care providers.
• Provide interventions addressing emotional, social, and financial consequences of illness or disability.
• Advocate for patient empowerment and autonomy in healthcare decisions.
• Collaborate with RN Case Managers, physicians, and families to plan discharge and care.
• Identify and mobilize community resources and services for patients and families.
