Job summary

Presbyterian Healthcare Services in Albuquerque, NM seeks a Medical Social Worker to evaluate, develop and implement discharge plans addressing psychosocial and financial needs of patients and families. You will collaborate with the interdisciplinary team to ensure safe community transitions and optimal outcomes.

Masters in Social Work and NM LICSW/LISW license required; prior clinical social work experience preferred.

Qualifications

• Master's in Social Work required; NM license as LISW or LMSW is mandatory.
• Experience or preceptorship in clinical social work desirable.

Responsibilities

• Identify and assess psychosocial problems; refer to appropriate services.
• Provide discharge planning and resource coordination for safe community transition.
• Document assessments and interventions in the medical record.
• Act as liaison to the multidisciplinary team and advocate for patients.
• Collaborate with team to clarify social work role and scope of practice.
• Serve as resource to colleagues and community case managers across regions.
• Promote effective communication among administration, staff, patients and families.
• Participate in policy, records, data and regulatory reporting as required.
• Complete MDS, RAPS, Care Plans and Utilization Review tasks.
