Job summary

Qualifications

• Experience coordinating care, assessments, and discharge planning with interdisciplinary teams.

Responsibilities

• Utilize assessment data to ascertain psychosocial risk factors and develop individualized treatment plans.
• Implement clinical interventions that promote mental health, rehabilitation and care transitions.
