UNM Hospitals in New Mexico is seeking a Social Worker for the OP Care Management Svcs team. You will assess psychosocial needs, coordinate with physicians, nursing and case management, and support timely, safe transitions across care levels.

Responsibilities include high-risk screening, discharge planning, care coordination, documentation, and liaison with community resources to improve patient outcomes. A Master’s degree in social work and licensure are required or pursued; CPR/BLS may be

Competenze

• Master's Degree in Social Work (MSW) or related field.
• NM LMSW / LPC / LCSW credentials a plus; active credential preferred.
• No minimum experience required.

• Manage psychosocial needs of patients and families to support medical conditions and transitions.
• Coordinate care with physicians, nursing, case managers and allied health staff to achieve outcomes within timeframes.
• Conduct high-risk psychosocial screening and ongoing assessments.

• Document data, problems, interventions and progress toward outcomes.
• Liaise with state protection agencies and legal counsel as needed.
