• Requires minimum of three (3) years of experience providing behavioral health, social services, or counseling support in a healthcare setting
• Requires demonstrated experience collaborating with interdisciplinary care teams to support discharge planning, care transitions, and continuity of care

Responsibilities

• Utilize assessment data to ascertain psychosocial risk factors and develop individualized treatment plans for each patient
• Responsible for implementing clinical interventions that promote, maintain or restore mental health, effect rehabilitation and facilitate developmental progression
