Job summary

Turning Point of Central California, Inc. is seeking an Intake Coordinator Mental Health Specialist Case Manager IV to support new client referrals and provide case management and rehabilitation services.

The role helps clients access Medi-Cal, primary care linkage, housing, employment services, and other supports. You will work with a multidisciplinary team to deliver integrated services, including medications, therapy, and community-based care, in a fast-paced environment.

Qualifications

• Bachelor's degree in social work, psychology, vocational rehabilitation, criminal justice, or related field.
• At least one year of experience in mental health or human services.
• Knowledge of best practices for Severe Mental Illness (SMI) and dual-diagnosis.
• English communication skills (written and spoken).
• Proficient computer skills in Office 365, Teams, Zoom.

Responsibilities

• Assist in the case management plan by screening, evaluating and implementing activities.
• Plan, coordinate and facilitate services for SMI clients.
• Work with a multidisciplinary team to support clients with housing, employment, health, and benefits.
• Provide direct services including case management and rehabilitation skills.
• Schedule intake assessments and coordinate enrollment.
