Turning Point of Central California seeks a Personal Service Coordinator/Case Manager II to assist individuals in accessing Medi-Cal eligibility, medical, social, housing, and educational services. The role collaborates with the PSC Supervisor and Assistant Program Director to implement the individual service plan and ensure effective service delivery.

Responsibilities include documenting case notes, coordinating referrals, and providing direct services per the service plan.

Qualifications

• Bachelor's degree in social work or counseling (required).
• Minimum of 1 year experience in a related field.
• Bilingual Spanish preferred; ability to communicate with diverse populations.

Responsibilities

• Assist in the case management plan for individuals under supervision by screening and evaluating needs.
• Maintain case records compliant with Medi-Cal guidelines and contract requirements.
• Attend weekly staff and supervision meetings to coordinate services.
