Job summary

Turning Point of Central California, Inc. in Fresno, CA is seeking an Intake Coordinator Mental Health Specialist Case Manager IV to support new client referrals, conduct intake, and connect individuals to Medi-Cal, healthcare, housing, and employment services.

You will work with a multi-disciplinary team to deliver integrated services, provide direct case management, and respond to crises. Bilingual capability (Spanish or Hmong) is valued; reliable transportation and a valid driver’s license

Qualifications

• Bachelor’s degree in social work, psychology, vocational rehabilitation, criminal justice, or related field.
• Minimum of one year of experience in a mental health or human services related field.
• First aid certification or training preferred.
• Knowledge of best practices for Severe Mental Illness (SMI) and dual-diagnosis.
• Effective written and spoken English communication.
• Computer skills in Office 365, Teams, Zoom, and related platforms.
• Valid driver’s license and reliable transportation.

Responsibilities

• Complete intake processes for new client referrals and coordinate enrollment.
• Plan, coordinate, and facilitate services for SMI clients and a range of supports.
• Work within a multi-disciplinary team to provide integrated services.
• Provide direct services including case management and rehabilitation skills.
• Maintain timely documentation and billing related to client services.
• Respond to crisis in the community and communicate with referral sources.
