Job summary

Turning Point of Central California seeks a Personal Service Coordinator/Case Manager III to support clients in office and community settings, helping them achieve daily living goals and independence. The role emphasizes case management, documentation, and collaboration with supervisors.

You will work with individuals with Severe Mental Illness and dual-diagnosis needs, using ISSP plans and navigating EMR systems, with required English and bilingual Spanish capabilities.

Qualifications

• Bachelor's degree in social work, psychology, vocational rehabilitation, criminal justice or related field.
• Minimum 1 year of rehabilitation case management experience.
• Knowledge of best practices for Severe Mental Illness (SMI) and Dual-Diagnosis.
• Proficient in written and spoken English.
• Ability to work with diverse groups and operate EMR systems.
• CPR and first aid certification ability.

Responsibilities

• Assist in case management plan activities to support client goals.
• Maintain case records per guidelines and complete timely documentation.
• Meet performance standards for direct services and required hours.
• Attend weekly supervision meetings with supervisor.
• Provide direct services including group and individual counseling per ISSP/Plan of Care.
• Drive on company business and assist with transportation as needed.
• Communicate with agencies and stakeholders involved in client rehabilitation.
• Provide timely reports on unusual occurrences affecting program operations.
