Job summary

US Heart and Vascular in Houston, TX seeks a Professional Fee Medical Coder, Level 3 to review documentation and assign CPT/ICD-10-CM codes for inpatient and outpatient encounters. The role emphasizes complex surgical coding, E/M encounters, and adherence to regulatory guidelines.

The coder will work with various EMR platforms, participate in physician queries, and ensure accurate charge entry and billing abstracts while maintaining ongoing education and compliance.

Qualifications

• High school diploma or GED required.
• 3 years of related experience required.
• 2 years of experience coding complex procedures preferred.
• Within 12 months of employment, one or more credentials required: RHIA, RHIT, CCS, CCS-P, CPC, or CCC.
• Proficiency in Microsoft Office and multiple EMR platforms.

Responsibilities

• Reviews encounter in a timely manner and resolves coding edits.
• Accurately assigns and sequences CPT, ICD-10CM, and HCPCS codes/modifiers per guidelines.
• Generates physician queries per established procedures.
• Provides feedback and education as required.
• Ensures compliance with USHV Coding Guidelines while coding.
• Performs charge entry of professional services.
• Abstracts information needed for billing.
• Performs charge reconciliation via logs and schedules.
• Meets coding quality and productivity targets.
• Completes all required education.
• Maintains continued education hours for credentials.
• Stays current with federal, state, coding, and departmental guidelines.
• Performs other duties as assigned.
