Job summary

Auburn Oaks Care Center is seeking an experienced MDS Coordinator to oversee the facility's resident assessments, including MDS and the plan of care, ensuring completion per guidelines. The role also tracks PDPM tools and coordinates weekly meetings for skilled patients.

The position trains new MDS Coordinators, monitors QM/QASP performance, and ensures timely completion and accurate data entry of MDS submissions while maintaining resident confidentiality.

Qualifications

• Graduation from an Accredited School of Nursing with BSN preferred.
• Must be in good standing with the State Board of Nursing and maintain continuing education/licensing.
• 3 years of supervisory experience preferred.
• Long Term Care experience preferred.

Responsibilities

• Oversee resident assessment process including initial assessments, MDS, and plan of care per guidelines.
• Provide oversight on weekly PDPM meetings for skilled patients and track PDPM tools usage.
• Monitor QM and QASP metrics and provide data-driven clinical recommendations.
• Provide PDPM and MDS training to new MDS Coordinators.
• Coordinate interdisciplinary participation in the MDS process and ensure regulatory time frames are met.
