Job summary

Superior HealthPlan, in Texas, is seeking an RN Care Coordinator to assess and coordinate medical and support services for complex/high acuity adult members. The role develops personalized care plans, educates families, and partners with care teams to ensure high-quality, cost-effective care across home health, hospice, and long-term care settings.

The position is hybrid with home visits and remote work from home. Requires 4–6 years nursing/case management experience; RN/BSN preferred.

Qualifications

• Graduate from an Accredited School of Nursing.
• Requires 4–6 years of related clinical nursing and/or case management and care coordination experience.
• Bachelor of Nursing degree is preferred.

Responsibilities

• Evaluates the service needs of the most complex or high risk/high acuity members and recommends a plan for the best outcome.
• Develops and continuously assesses ongoing long-term care plans / service plans and collaborates with care management team to identify providers, specialists, and/or community resources needed to address member's needs.
• Coordinates and manages between the member and family/caregivers and the care provider team to ensure members are receiving appropriate person-centered care.
• Monitors care plans and member status, revising plans as necessary to meet goals/needs.
• Reviews member data to identify trends and improve operating performance and quality care.
