Job summary

Distinctive Living seeks an LPN for Mira Vie at Tinton Falls in Tinton Falls, NJ. The role emphasizes supervising care staff, maintaining care plans, and ensuring regulatory compliance in a warm, resident-focused community.

You will collaborate with the Health and Wellness Director, monitor MARs, and provide hands-on nursing care while upholding HIPAA and safety standards in daily operations.

Qualifications

• LPN licensure required.
• Must have one year of nursing or caregiver experience, preferably in assisted living, home health, or memory care.
• Minimum six months of supervisory experience.
• Experience with Microsoft Word and Excel preferred.
• Must be able to read physician orders and match to MARs.
• Understanding of medical terminology adequate to pass medications.
• Excellent verbal and written communication skills.
• Able to communicate verbally in person and over the phone.

Responsibilities

• Follow the resident service plans and document any changes in condition per regulations.
• Communicate resident needs to the Director of Health and Wellness as needed.
• Provide hands-on care and support the care plan; review plans for regulatory alignment.
• Provide clinical support and staff training for continuing education.
• Transcribe and review physicians' orders to ensure accuracy.
• Maintain required certifications and participate in ongoing trainings.
• Support CQI programs and report issues impacting quality of service to leadership.
• Review MARs and Daily Reports to ensure appropriate clinical interventions.
• Follow HIPAA, OSHA, and Bloodborne Pathogen guidelines.
• Promote safety, teamwork, and professional conduct across staff and residents.
• Perform other duties as assigned.
