Job summary

Distinctive Living is seeking a full-time LPN/LVN at our Brooklyn Pointe Community. The role focuses on assessing resident health, developing care plans, and ensuring regulatory compliance while collaborating with the Director of Health and Wellness and care staff.

You will provide hands-on nursing support, review MARs, and train staff to meet state guidelines. Growth, supportive culture, and comprehensive benefits are offered.

Qualifications

• High School Diploma or equivalent required.
• Must have one year of nursing/caregiver experience in assisted living, home health or memory care.
• Minimum six months supervisory experience.
• MS Word and Excel experience preferred.
• Must be able to read physician orders and match to MARs.
• Medical terminology knowledge to communicate with professionals.
• Excellent verbal and written communication.
• Able to communicate in person and on the phone.

Responsibilities

• Follow resident service plans and document changes of condition.
• Communicate resident needs to the Director of Health and Wellness.
• Provide hands-on care and nursing tasks as required by the care plan.
• Assist in transcription of physicians' orders and review new orders.
• Maintain required certifications and in-service trainings.
• Support CQI program and report quality issues to leadership.
• Adhere to HIPAA, OSHA and Bloodborne Pathogen guidelines.
• Work safely and model professional behavior.
