Distinctive Living's Mira Vie at Clifton in Clifton, NJ is seeking a part-time LPN/LVN to provide resident care and support under the Health and Wellness Director. The role focuses on assessing health status, developing and updating care plans, and ensuring regulatory compliance within the community.

You will collaborate with direct care staff, medication aides, and nursing teams, maintaining accurate documentation and contributing to a caring, growth-oriented culture.

Formación

• High School Diploma or equivalent required.
• LPN/LVN licensure required.
• One year of nursing or caregiver experience preferred (assisted living, home health or memory care).
• Minimum six months supervisory experience.
• Microsoft Word and Excel experience preferred.
• Must read physician orders and match to MARs.
• Understanding of medical terminology for communication with medical staff.
• Excellent verbal and written communication skills.
• Able to communicate in person and over the phone.

Responsabilidades

• Follows services outlined in each resident's service plan and documents changes of condition per local rules, regulations, and policies.
• Communicates resident needs to the Director of Health and Wellness as needed to ensure quality services.
• Participates in hands-on care and nursing tasks to support residents' care plans and state regulations.
• Provides clinical support and training to care staff for continuing education requirements.
• Assists in transcription of physician orders and review of new orders.
• Maintains required certifications and completes ongoing in-service trainings and accreditations.
• Supports CQI Program and ensures quality services are delivered.
• Ensures compliance with HIPAA, OSHA, and Bloodborne Pathogen guidelines.
