Job summary

Vinfen is seeking a qualified LPHA Clinician to promote integrated community-based care for Persons Served, coordinating intake, assessment, treatment planning, and interventions. The role leads a multidisciplinary team, mentors staff, ensures adherence to evidence-based practices, and supports transitions in care to maximize recovery outcomes.

Qualifications

• Master’s degree required in Social Work, Psychology, Counseling Psychology or LPHA licensure eligible program.
• LCSW, LICSW, LMHC, LRC, LADC 1, OTR, RN or Licensed Psychologist in state of service required.
• Clinical experience working with people with psychiatric disability and/or co-occurring disorders preferred.
• Must have access to an operational and insured vehicle and be willing to use it to transport PS.
• Bilingual/bicultural applicants and applicants with lived experience of psychiatric conditions are encouraged to apply.

Responsibilities

• Initiate and conduct intake and engagement with PS within 72 hours.
• Engage and assess treatment and support needs of PS.
• Lead treatment plan meetings with PS and involve stakeholders.
• Oversee service delivery and progress across PS, including transitions.
• Provide field supervision to Outreach Workers and staff delivering Treatment Plans.
