Job summary

Twin Cities Physicians is seeking an Advanced Wound Care Nurse Practitioner to serve senior living communities throughout the Twin Cities Metro. This role combines advanced wound care, community rounding, chronic disease management, and geriatric medicine into a practice environment that few providers ever experience.

You'll work directly within assisted living communities, memory care settings, skilled nursing facilities, and long-term care environments, helping patients receive specialized

Qualifications

• Active Minnesota Certified Nurse Practitioner (CNP) license (AGNP preferred).
• Experience in wound care, geriatrics, primary care, post-acute care, or skilled nursing preferred.
• Strong clinical judgment and confidence managing medically complex patients.
• Valid driver's license and ability to travel throughout the Twin Cities Metro Area.

Responsibilities

• Provide advanced wound care assessments, treatment planning, and ongoing management of complex wounds.
• Conduct routine rounding visits throughout assisted living, memory care, skilled nursing, and long-term care communities.
• Manage medications, diagnostics, treatment plans, and follow-up care.
• Collaborate with physicians, nurses, therapists, caregivers, and facility leadership.
• Support chronic disease management, preventative care initiatives, and continuity of care.
• Help improve outcomes while reducing avoidable complications and hospitalizations.
