StoryPoint in Valparaiso, IN seeks a Wellness Director to lead clinical resident care services, delivering a 1440 experience and optimizing performance while strengthening team connections.

You will hold an Indiana LPN or RN license, manage revenue and care plans, coordinate dementia programs, and ensure regulatory compliance across the community. This role requires flexible scheduling including weekends and holidays and offers comprehensive benefits.

Qualifications

• Licensed Practical Nurse (LPN) or Registered Nurse (RN) licensed in Indiana.
• CDP required within 90 days of hire.
• Assisted Living/Memory Care experience preferred.
• Leadership experience preferred.
• Ability to manage revenue and financials.
• Proven high performer.

Responsibilities

• Lead and be accountable for your team and performance.
• Drive revenue for Wellness, Evaluations & Care Conferences and Service Plans.
• Coordinate Alzheimer’s and Dementia Program activities.
• Align with 1440 care standards across the community.
• Clearly communicate vision to staff, residents, and partners.
• Foster partnerships with community leaders and external partners.
• Market wellness with Executive Director and Sales Team.
• Perform resident admissions and semi-annual evaluations.
• Ensure state regulations and audits are met.
• Obtain written orders for necessary changes to care plans.
• Verify MARs and coordinate with pharmacy for meds.
• Review incidents and address staff issues as needed.
• Communicate emergency policies and updates to staff.
• Pursue continual improvement of the organization.
• Maintain flexibility for on-call, weekends, and holidays.
• Perform other duties as assigned.
