Job summary

Catholic Housing & Community Services is seeking a full-time Social Services Care Manager to oversee caseloads at two elderly housing sites in Collingdale and Eastwick, Philadelphia. You will engage residents, assess needs, develop care plans, coordinate services, maintain records, and promote holistic, age-friendly programming within a supportive community.

A Master’s in Social Work or Gerontology and at least three years’ experience with older adults are required; this role is with an equal

Qualifications

• Master's degree in Social Work or Gerontology required.
• Minimum three years' experience with older adults.
• Knowledge of federal and state benefits programs.

Responsibilities

• Manage caseload of older adult households in two apartment communities.
• Assess residents for aging in place, including health, daily living, mobility, and benefits.
• Develop resident-centered care plans with interventions and resources.
