Job summary

Philadelphia Corporation for Aging (PCA) is seeking an Aging Care Manager I for our PCA Main Building location. This full-time role conducts ongoing assessment, service planning, and care coordination for disabled and chronically impaired older adults to improve quality of life.

The position requires a BA/BS/BSW or MA/MS/MSW with 12 college credits in sociology, social welfare, psychology, or gerontology, and at least 1 year in social work or care management related to aging, behavioral health,

Qualifications

• BA/BS/BSW or MA/MS/MSW with at least 12 college-level credits in sociology, social welfare, psychology, or gerontology.
• Minimum one-year work experience in social work or care management in aging, behavioral health, or disabilities.

Responsibilities

• Maintain high-quality care management services and provide home and community-based services to enable aging in place.
• Perform assessment, service plan development, service arrangement, follow-up and reassessment.
• Complete NAT and participant care plan within required timeframes.
• Enter Care Management Activity in the system.
• Collaborate with participant, family, and caregiver to develop an individual care plan.
• Mobilize informal and formal resources to meet needs; maximize third-party payers.
• Facilitate provider choices and arrange services/entitlements; follow up on service delivery.
• Assist with MA Application Enrollment and Community Health Choices processes.
• Conduct home visits and calls to monitor plan adequacy.
• Monitor participant satisfaction and maintain records in databases.
• Participate in orientation, supervision, and professional development.
• Support a collaborative, supportive team culture and perform other duties.

Education

BA/BS/BSW or MA/MS/MSW incl. 12 college credits in sociology, social welfare, psychology, or gerontology

Minimum 1 year experience in social work or care management (aging/behavioral health/disabilities)
