Job summary

National Church Residences, the nation’s largest provider of affordable senior housing and health care services, seeks Service Coordinators for Solomon Heights in Queensbury, NY. This part-time role (20 hours/week) focuses on coordinating resident programs, connecting with community services, and delivering excellent resident support.

The ideal candidate has a BS in social work or related field and 2+ years of elderly/family services experience, plus strong advocacy and organizational skills.

Qualifications

• Bachelor’s degree in social work or related field preferred.
• Two+ years of experience delivering social services to elderly or family populations preferred.
• Working knowledge of supportive services and local community resources.
• Ability to advocate, organize, problem-solve, and deliver measurable results for residents.

Responsibilities

• Advocate for residents and organize services to meet needs.
• Coordinate programs and activities for residents.
• Liaise with community agencies and negotiate discounts or identify low-cost providers.
• Engage residents to identify needs and refer to community resources.
• Develop a Resource Directory of state/local service providers.
• Sponsor educational events on health care, support services, and life skills.
• Provide quality customer service and assist with basic needs.
• Help residents access housekeeping, meals, transportation, personal services, or financial assistance.
• Monitor and relay feedback from residents and families.
• Manage relationships with internal and external stakeholders.
