Qualifications

• Bachelor's degree in social work or related field preferred.
• Two+ years of experience in social services for elderly/family population.
• Ability to advocate, organize, problem-solve, and deliver results.

Responsibilities

• Coordinate programs and activities for residents.
• Serve as a liaison to community agencies and providers.
• Identify low-cost services and negotiate discounts.
• Engage residents to identify needs and refer to services.
• Develop a Resource Directory of providers.
• Sponsor educational events on health care, support services, and life skills.
• Provide quality customer service addressing concerns and assisting with needs.
• Assist residents in accessing transportation, meals, housing, and other services.
• Report complaints and manage relationships with internal/external partners.
