Job summary

Reform Rehab | Pegasus Landing of Chisholm Trail is seeking a Physical Therapist (PT) and/or Occupational Therapist (OT) to provide mobile outpatient therapy at Pegasus Landing in Keene, Texas. The role serves seniors in an assisted living setting with flexible weekday hours and consistent local coverage.

Responsibilities include evaluations, treatment, documentation, and coordination with facility staff. Texas license required; CPR preferred; new graduates welcome to apply.

Qualifications

• Current Texas Physical Therapist or Occupational Therapist license in good standing.
• Graduate of an accredited PT or OT program.
• CPR certification preferred.
• Experience with older adults or senior living is a plus.
• Strong communication, organization and documentation skills.

Responsibilities

• Provide skilled PT and/or OT evaluations and treatments based on discipline.
• Develop and implement individualized plans of care.
• Treat older adults with mobility, strength, balance and ADL limitations.
• For PTs: therapeutic exercise, gait training and functional mobility interventions.
• For OTs: therapeutic activities, ADL training and functional training.
• Complete timely documentation and ensure Medicare compliance.
• Communicate with patients, families, facility staff and Reform Rehab team.
• Coordinate care with other therapy disciplines when applicable.
• Maintain a professional and positive presence in the senior living community.
