Job summary

Reform Rehab is seeking a Physical Therapist (PT) and/or Occupational Therapist (OT) to provide mobile therapy coverage at Pegasus Landing of Chisholm Trail in Keene, TX. You will deliver evaluations and treatments, develop plans of care, and work primarily with older adults in a senior living setting.

Qualifications

• Current Texas PT or OT license in good standing.
• Graduate of an accredited PT or OT program.
• CPR certification preferred.
• Experience with older adults or senior living is a plus.
• Strong communication, organization and documentation skills.

Responsibilities

• Provide skilled PT and/or OT evaluations and treatments based on discipline.
• Develop and implement individualized plans of care.
• Treat primarily older adults with mobility, strength, balance, gait and ADL limitations.
• For PTs: therapeutic exercise, gait training, balance training and related interventions.
• For OTs: therapeutic activities, ADL training and functional training.
• Complete timely documentation and comply with Medicare regulations.
• Communicate with patients, families, facility staff, and Reform Rehab team.
• Coordinate care with PTs, OTs, PTAs, and COTAs as applicable.
• Maintain a professional presence within the senior living community.
