Job summary

Reform Rehab is seeking a Physical Therapist and/or Occupational Therapist to provide mobile outpatient therapy at Pegasus Landing of Chisholm Trail in Keene, Texas. The role focuses on delivering skilled PT/OT evaluations and treatments to older adults in a senior living setting.

Typically weekday hours with one-on-one sessions. Compensation includes $60 per completed patient visit and opportunities for growth within the expanding outpatient company.

Qualifications

• Current Texas Physical Therapist or Occupational Therapist license in good standing.
• Graduate of an accredited PT or OT program.
• CPR certification preferred.
• Experience working with older adults or in a senior living setting is a plus.
• Strong communication, organization, and documentation skills.
• Reliable and comfortable working independently.
• New graduates are welcome to apply.

Responsibilities

• Provide skilled physical therapy and/or occupational therapy evaluations and treatments based on the therapist's discipline.
• Develop and implement individualized plans of care.
• Treat primarily older adults with mobility, strength, balance, gait, functional, orthopedic, and activities-of-daily-living limitations.
• For PTs: provide therapeutic exercise, neuromuscular re-education, gait training, balance training, functional mobility training, and other appropriate skilled interventions.
• For OTs: provide therapeutic activities, ADL training, functional training, neuromuscular re-education, and other appropriate skilled interventions.
• Complete timely and accurate documentation and maintain compliance with Medicare and applicable regulations.
• Communicate effectively with patients, families, facility staff, and the Reform Rehab team.
• Coordinate care with other therapy disciplines, including PTs, OTs, PTAs, and COTAs as applicable.
• Maintain a professional and positive presence within the senior living community.
